How to Register

New Patient Registration


If you wish to register with the Practice, please come along to reception between 10.00am and 5.00pm, and ask for a registration form. You will normally be able to register on the day. The Practice accepts new patients from within the city boundary east of the River Exe, excluding Topsham. Click here to check if your address is in our area

If you have any of the following information available, this might be useful when completing the registration forms:
  • Proof of address( eg: bank statement, utility bill, driving licence) 
  • NHS number (You can get this from your previous GP Practice)
  • You will also need to know your previous GP Practice name and address. 
  • If you are coming from abroad, your EHIC card or your Surcharge paperwork
When you attend the surgery to complete your registration forms we will ask you to complete a mini health check. This is an opportunity for us to obtain details of any pre-existing medical conditions such as diabetes, asthma, high blood pressure (hypertension), etc. and determine if you have any current problems.  

Your Registered GP

All patients registered with The Heavitree Practice will have a named accountable GP. Patients are told who their GP is when they register and are kept informed if this changes eg when a GP leaves or joins the Practice. If you are not sure which GP you are registered with, please ask at reception.

Your GP will take lead responsibility for your care, including coordination of any healthcare services they judge that you require. However this does not mean that you can only see this GP, particularly if you need an appointment on a day or at a time that they are not available.

To ensure that our GPs’ caseloads are balanced we have one GP to whom we assign new patients each month. We also try to keep immediate family members together.  If you have a strong preference as to which GP you are assigned please let reception know and we will make reasonable efforts to accommodate your request.

Registering online 

If you are a student with Exeter university DO NOT use the online registration form, you will need to complete a physical registration form and hand them in at the practice reception. For all other registrations you can register online using the link below. You will need to come into the practice within 7 days of submitting the online form to complete the registration. You will need to bring with you with proof of address and, if you would like to sign up for online access to you records, Photo ID.
